Fort Campbell recently launched an assessment aimed at collecting information pertaining to the health and readiness of the military community.
  
Soldiers, family members, Army civilians and military retirees can participate from the comfort of their home or office by clicking the Community Strengths and Themes Assessment (CSTA) link at https://usaphcapps.amedd.army.mil/Survey/se.ashx?s=251137452E7FC2CD.
  
The CSTA is designed to capture the military community's perceptions of the quality of life, health, safety and satisfaction within the Army installation environment. The information will be used to help assess community needs for program planning and policy development, and for program evaluation.

Responses are confidential, assured under Section 308(d) of the Public Health Service Act. Additionally, procedures are in place to prevent the disclosure of personal data, including data encryption and secure data networks. No personal identifiers are collected as part of the assessment process. Responses are collected by the U.S. Army Public Health Center and securely transmitted to Fort Campbell to be used for identification of public health concerns, monitoring community perceptions, evaluation and statistical purposes.
 
The assessment will be open through May 30 and takes 10-15 minutes to complete. Findings will be available by the end of the summer 2019.
